Trying to get medically cleared for a sport at York College?
 
Prior to participating in a sport, all student-athletes must have the Pre-Participation Medical Forms filled out by a doctor, as well as complete the information in the SWOL online data entry system: https://www.swol123.net.   Please be sure to read the Sports Medicine Student-Athlete Medical Testing Policies as well. All forms and instructions are listed below.

  • Please fill out the form and have it mailed to you.  They do not email the form. It can be faxed if you have access to a fax machine. 
    Please add that you are requesting these results for the NCAA Sickle Cell Requirement.
